Job Description:
A company is looking for a Care Manager RN - National Remote to
provide leadership in the coordination of patient-centered care
across the continuum.
Key Responsibilities
Coordinate patient care processes to achieve quality outcomes and
control resource utilization
Facilitate patient and family education, ensuring continuity of
care and optimal patient outcomes
Integrate assessments for post-hospital services and develop
appropriate discharge plans for complex cases
Required Qualifications
Associate's Degree (or higher) in Nursing
Current, unrestricted RN license in the state of residence
3+ years of experience in a hospital, acute care, or direct care
setting
Intermediate level of proficiency to type and navigate a
Windows-based environment
Background in managed care and case management experience
preferred
